Per Diem Social Worker or Case Manager
Edgerton Hospital and Health Services is accepting applications for a per diem Social Worker or Case Manager. The Social Worker is responsible for providing for the psychosocial assessment, plan and medication as determined by the patient needs and medical direction. This candidate must possess a minimum of a Masters Degree in Social Work from an accredited school, at least two years experience and a valid Social Work License in the State of Wisconsin.
The Case Manager is a registered nurse who has a background in medical/surgical nursing and case management and who collaborates with the medical staff, interdisciplinary team, and the Director of Nursing on issues related to the managment and effectiveness of patient care. This candidate will have an RN license in the State of Wisconson, BSN required or BSN in Case Management, and three years clinical experience.
Estimated Salary: $20 to $28 per hour based on qualifications.
